Looking for a js programmer or someone with knowledge appliying/customizing js codes of a from page & confirmation page. Most of the codes are already published on many js related sites.
The codes to use are:
1 - onchange (drop down menu); disable,enable (text input fields and a drop down menu)
2 - Luhn credit card validation
3 - Input text field copy (first name, last name)
4 - Agree to terms check box (disable submit)
5 - Customize confirmation page (display: first name, last name, phone #, email address, pickup location, drop-off location, flight #, airline, pickup date & time).
$25.00/hr and DON'T want an estimate, just a fix rate (if it takes you more/less time I don't want to be charged more, but won't pay you less)
